Our short-term rehab professionals create a custom program to help restore the highest level of functioning possible, as soon as possible. While receiving medical, therapeutic, and social services, we also provide the little things that can make a huge difference – a good meal or a patient ear.

- for discharge to the community. This measures whether a beneficiary remains in the community without re-admission to a hospital or Skilled Nursing Facility.
- for Medicare spending per beneficiary. This measures total Medicare dollars spent during the Skilled Nursing Facility stay and for 30-days post discharge.
- for potentially preventable 30-day post-discharge re-admission measure. This measures potentially preventable unplanned hospital re-admissions for Medicare beneficiaries within 30-days of prior proximal acute care, critical access or psychiatric hospital stay.
We know the rules and regulations and can often ensure your treatment continues, so that you can make a full recovery. We even follow up after your short-term rehab to arrange visiting nurses, home health aides or other services you may need once you go home.
